PHOTO/VIDEO/WEBSITE RELEASE FORM
Dear Parent/Guardian,
On occasion, representatives of Peachtree City First Baptist Church (the “Church”) may photograph, videotape, and/or interview adults and children in connection with the Church’s programs or events. The entire community benefits from knowing about the Church’s services and the programs offered by the Church to children and families.
In order to enable the Church to release any student photos, video footage, comments and/or interviews for publicity purposes and to post such items on the Church’s website and social media, we need your written permission and consent. Please sign and date the form below.
I give permission for my child to be photographed or videotaped for use and reproduction by First Baptist Church Peachtree City without compensation to me/my child. All photographs/video tapes or other recordings of any kind shall be the property, solely and completely, of the Church. I hereby waive any and all rights to inspect or approve the finished photographs,videotapes, and/or the sound track, script or printed material that may be used in connection with such photographs and/or video recordings.
This release and consent shall remain valid and in full force and effect until notice of its revocation shall be received by the Church.
